y2="14"></line></svg></button></div></div></div></a></figure></div><h2>There was a year I could not trust my own brain.</h2><p>I am going to start today&#8217;s story with that year because everything I now believe about healing evolved from that year. I see patients every week in that same capacity collapse.</p><p>My identity has always lived in my mind. Pattern recognition is what I do. Read the labs, see the connections between the body&#8217;s systems, find the sequence that maybe four other doctors missed. For 25 years that was the most reliable thing about me. <strong>Then long COVID and POTS challenged that.</strong></p><p>I could not walk into REI during the holiday season. The lights, the sound, the holiday shopping decisions stacked on shelves, all of it hit my nervous system that had lost its ability to filter, and I would be flooded with overwhelm. That year I started paying for business class seats on flights. Not for the legroom. Economy seats had more sensory input than my brain could process in that year. At altitude I could feel solar storms move through my nervous system, which is the kind of sentence I would have thought to be overly dramatic if a patient shared that with me. I went hypoglycemic on a stationary bike, watching my glucose fall into the 50s during a workout my body had done a thousand times without this level of crash.</p><p>Here is the irony in this story.  I am an advanced functional medicine doctor.</p><p>By then I was trained at the advanced edge of functional medicine. I knew the peptide protocols. I knew hormone optimization. I had access to every stimulating, sophisticated tool this field offered, and I understood the mechanisms better than most people prescribing them. If stimulation and sophistication could have rescued anyone, it should have been me. I reached for the advanced tools first, because that is what advanced training conditions you to do. I needed the fix and fast. </p><p>My system interpreted every sophisticated and potent treatment as a threat and escalated into more chaos.</p><p>What worked was almost insulting in its simplicity. </p><p>Sleeping in pitch darkness protected me like a baby. Breath work, the slow nasal kind that builds parasympathetic tone, not the kind that has become its own performance sport. Immune balancing before any immune stimulation. Eating enough, containing energy instead of spending it, building muscle to carry more of my metabolism. And waiting. </p><p>And waiting.</p><p>Tolerating uncertainty at a time I did not choose. This is exactly what I ask my most complex patients to do and exactly what I hated doing myself.</p><p>Sophistication was never the missing piece. Sequence was.</p><h2>What the ceiling looks like from my clinical perspective.</h2><p>I sit across from the people treating the ceiling every week.  It shows up in three types of cases.</p><p>The first is data. A woman came to me with three years of Oura history, a CGM subscription, two GI-MAPs, a DUTCH panel, a full methylation workup, and a readiness score she is trying to perfect like a test and failing. Hundreds of pages of information. She is watching all her failing tests but still no answer as to why she cannot carry groceries up a flight of stairs without having to recover with a nap.</p><p>All of that measurement, and every number was a number about output. A readiness score is still a ceiling number. Nothing in her stack of data explained why the numbers refused to move. The answer was in the interpretation of her wearable and pages of data overlooked: the gut lining her immune system was defending against, the cell membranes where signals were failing to pass, the nervous system that had not left survival mode in four years. She had quantified the ceiling with data and precision. Nobody had recognized the faulty floor in the interpretation of her data.</p><p>The second common case is hormones. A perimenopausal woman arrives with an estradiol patch and a testosterone pellet; her levels pushed to the top of the range because someone told her that was optimal. Her sleeping is fragmented and she wakes wired at 2 a.m., more anxious than before the hormone protocol started. Hormones are signals. They act on the body&#8217;s terrain. Amplify a signal into a nervous system that cannot hold stability and you do not get vitality. You get louder chaos, delivered consistently, at a specialty hormone price.</p><p>The third is peptides. A repair stack for gut and brain fog, layered onto a histamine-dominant immune system that reacts to nearly everything it is given. A hypervigilant immune system does not heal from potent treatments like hormones or peptides. It reacts to them. Then the reaction gets treated with another supplement.</p><p>Overstimulated. Overpriced. Overpromised. That&#8217;s my unedited thoughts after a hard clinic week. Not because I think the treatments are wrong. Peptides have real pharmacology. Hormone therapy transforms when the terrain is ready for it. These are advanced tools, and advanced tools are not the problem. The problem is the order.  </p><p>The problem is that basic has to come before sophisticated, and we often set up our care staffing exactly backwards. The floor gets handed to a nutritionist or a health coach, as if rebuilding immune tolerance and receptor function were lifestyle advice, while the medical provider swoops in with the peptides and hormones at the end. This is the wrong order. The most medically demanding phase of the floor went to lifestyle and general advice, and the physicians were saved for the part that only works after the hard part is done-building the floor.</p><h2>The floor, and why nobody sells it</h2><p>The floor is a single layer of cells lining your gut, deciding what enters your body and what stays out. It is the immune tissue beneath that layer, making tolerance decisions thousands of times a day: defend or repair. It is the cell membrane, built from the fats you eat, and the receptors embedded in it that determine whether a hormone&#8217;s message gets heard at all. This is the architecture and organization every sophisticated intervention has to signal through and connect. </p><p>When the floor is compromised, there is no version of optimization that holds, because there is nothing for it to hold onto.</p><p>No one sells the floor. Nobody makes a Reel about membrane composition. It does not produce a two-week transformation story, and that is precisely the issue, because the epithelial cells turn over in days but the body&#8217;s terrain that supports them, the immune tolerance, the lipid architecture, the microbial ecosystem, can take months to years to heal.</p><p>Nobody wants to hear that it will take that long.</p><p>So patients are handed a six-week protocol. They feel the early flash of stimulation, hit the wall where the floor should have been there to hold them, and conclude that the whole functional medicine model failed them. Most might quit right there, a few months short of the place they were trying to reach. They did not fail the process. The process didn&#8217;t spend enough time building the foundation, the floor.</p><p>The committed ones stay and get misled differently. They keep buying: the supplement stack that grows past twenty bottles, the next peptide, the hormone adjustment, sophistication layered on sophistication with no foundation under any of it. That is the expensive mistake I watch most often, and it is the mistake I made in my own body with the all my advanced knowledge pushing through with desperation for some relief.   </p><p>I learned to go back to the floor work. Complete that work.</p><h2>Recovery is the floor with a number on it</h2><p>We measure VO2 max in my clinic, and after measuring multiple patients I was seeing a different pattern that I did not expect. The elite athlete is not the one with the highest peak output. It is the one whose system returns to baseline fastest and most completely between efforts. Recovery capacity is measurable, trainable, and it is the true limiter of performance. Push output without it and you get a body headed toward breakdown that many patients feel as the price of healing.</p><p>The ceiling of performance is set by the floor of recovery. Health follows that same law.</p><p>So at Root-Cause we measure the boring side. Recovery, not just output. Restoration, not just loading. The yin and the yang of physiology, and I say that knowing it sounds flaky, but it turns out to be the most empowering support we provide. It is where the people who had given up finally start to hold their healing ground instead of banging their head on a ceiling only to crash back down.</p><h2>Why I hold this work the way I do</h2><p>People choose Root-Cause after investing in the ceiling. After the peptides, after the optimization, after the hundreds of pages of data that never explained anything. They are not wrong or a failure, because I know exactly what it cost them to keep trying.  I have had to pay that same price. </p><p>Most of what we do comes down to this: I aim to help a person build their floor and foundation sturdy enough to go live the next chapter of their life, the work, the marriage, the kids, the things they want, instead of spending every bit of capacity just to get through a day. I know from the inside what it is to lose that capacity and the long and enduring work it takes for it to return.</p><p>That is sacred work. She was like a &#8220;<em>curandera&#8221;</em>. She read plants by feel and smell, and she read the body the same way: as one system, where a gut problem could be a nervous system problem could be a hormone problem. Nobody had to teach her that. </p><p>Plant, person, land, connected without a seam.</p><p>I grew up believing that was simply how medicine worked.</p><p>Then I trained in medicine myself. Twenty-five years of clinical practice, over a decade at the advanced edge of functional medicine. What I found inside the conventional system was the opposite of what I came from: the body sorted into specialties that never spoke to each other, chaos managed with pharmacy, a patient handed from one department to the next while no one asked why the pattern kept repeating.</p><p>The labs looked fine. The pattern never was normal.</p><p>That gap is where Root-Cause Medicine started. Not as a brand position. </p><p>As the only honest response to what I kept watching happen in the exam room.</p><h2>What both systems get wrong</h2><p>I expected functional medicine to be the answer, and for a while it was closer. Better questions. Better labs. A willingness to look upstream.</p><p>But most of functional medicine now runs on the same operating system as the model it left. A diagnosis gets mapped to a supplement protocol the way a diagnosis gets mapped to a prescription. Patients arrive in my clinic having received probiotics and fermented foods before anyone asked whether their immune system could tolerate them. Perimenopausal women get hormone optimization layered onto a nervous system that cannot hold the potency of a hormone without stability. The wellness world sells the ceiling, longevity stacks and heavy stimulation and optimization, to people who have never been helped to build a floor.</p><p>More support is not always better. Better sequencing is better.</p><p>That is the entire argument of my clinical life, and I did not learn it from a certification. I learned it in my own body. I have personally worked through burnout, HPA axis dysfunction, metabolic resistance, and a capacity collapse severe enough that I could not think my way out on my own timeline. I had to wait, support the system, and tolerate uncertainty (and fear), which is exactly what I ask my most complex patients to do. When I sit with someone who has been dismissed by four doctors and told her labs are normal or that they need another supplement stack, I am not performing empathy. I know the body&#8217;s terrain and interconnectedness.</p><h2>What I built instead</h2><p>The INSIGHT&#8482; Method is how I organize what my grandmother understood intuitively into something measurable, teachable, and repeatable: seven systems (Inflammation, Nutrients, Stress and the HPA axis, Insulin and Metabolism, Gut Health, Hormones, Toxins) read as one conversation instead of seven separate problems. <strong>The work is interpretation and order of operations.</strong> Which system is driving. Which intervention the body is actually ready for. What has to stabilize before anything gets stimulated.</p><p>Precision medicine with ancestral roots. Laboratory data read the way my grandma read plants: not in isolation, but in a conversation with everything else.</p><p>My medical school application essay was about my grandmother. It took over 25 years to build what I was writing about and witnessing.</p><h2>What this Substack is</h2><p>This is where I write the longer arguments. The clinical patterns I have watched repeat for 25 years that neither conventional nor functional medicine has language for. Why fatigue is the most common complaint in medicine and the least understood. Why the low-capacity patient has quietly become the new clinical baseline. Why hormones are signals acting on terrain, and what happens when you amplify a signal in an unstable system. <strong>What actually changes when a body starts to heal in the right order.</strong></p><p>I am done convincing skeptics. This is not written to defend a model. It is written for the people who already suspect the pattern is real and has been misinterpreted: patients who have been told everything is normal while everything is wrong, and for clinicians who feel overwhelmed by the ceiling of protocol-based care and want the interpretation layer nobody taught them.</p><p>I will publish every other week.
